Nephrogenic Diabetes Insipidus

Definition

Nephrogenic diabetes insipidus is a disease characterized by the production of excessively dilute urine due to the kidneys' failure to respond to antidiuretic hormone (ADH).

Symptoms

  • Excessive urination (polyuria)
  • Excessive thirst (polydipsia)
  • Dehydration
  • Fever (in infants)

Reasons

Hereditary (V2 receptor mutation) or acquired (lithium use, hypercalcemia, hypokalemia).

Diagnosis

Inability to concentrate urine on water deprivation test, high plasma osmolarity.

Treatment

Adequate fluid intake, thiazide diuretics, low salt and protein diet.
